PFM Instructions

WARNING: Please note that the AC500 and AC660 sets look the same. You MUST READ the labels carefully so that you use the AC500 bottle #1with the AC500 #2 together and the AC660 bottle #1 with the AC660 bottle #2 together. Do not mix AC500 with AC660.
Each kit contains 2 sets of foaming agents (one set of AC500 and one set of AC660), a 7ft. bag and assorted pieces.
SAFETY FIRST: Always practice safety for your patients and staff. Wear protective gloves, smock and eye protection. Have patient remove jewelry before making devices. Always follow directions!
INSTRUCTIONS
1. Comfort your patient. Tell your patient what to expect. They must lay still for 15 minutes while their personal customized form rises around them. It will feel comfortably warm as the foam rises and hardens in the position they will need to hold while being treated.
2. Open the 7 ft. bag and roll back the opening as shown. Attach the white end piece, the footrail, onto the blue backboard. Next, attach the side pieces to the desired width. Finally, attach the knee rests to support the patient’s knees.

3. After preparing the device, you are ready to mix the foaming agents and pour into the device. Have one person mix the AC500 foaming agents and another person mix the AC660 foaming agents at the same time. Remove the caps and pour ALL of bottle #1 into bottle #2. Cap bottle #2 and shake with the cap pointing down, over the device, for ONLY TEN SECONDS. Immediately remove the cap and pour the entire mixture into the device as shown. Make sure there is plenty of foam on the knee supports. Dispose of the bottles in a leakproof container. DO NOT RECAP.

4. After pouring the foam into the device, carefully unroll the bag and place the form deep into the bag. Next, press the bag against the foam and remove as much air from the bag as you can. Lift the end of the device and tuck the bag underneath. This will contain the foam and prevent any foam from leaking out of the bag.
5. As the foam begins to rise, CAREFULLY guide the patient into the device and into the setup position. The patient must remain motionless for 15 minutes. After removing patient from device, let form remain on the treatment table for 10 minutes. Your device is now ready to use.

HELPFUL HINTS
USE TWO PEOPLE TO MAKE THIS DEVICE: This device uses two different sets of foaming agents, the AC500 and the AC660. It is impossible for one person to mix both sets of foam at the same time. Use two people so that the sets of foam can be mixed at the same time. Remember to remove the caps after TEN TIMED SECONDS! Do not leave the cap on one set while pouring the other set of foam into the device. Pour both sets of foam into the device at the same time. This will prevent accidents with the foam. Squeeze as much foam as possible out of the bottles.
CREATING TIGHT, SECURE DEVICES: When making the customized device, you must make deep impressions and keep them tight against the patient during the foaming process. This is the only way to effectively create a worthwhile anatomical device. As the foam rises above the mold, press down hard and hold the foam down tight against the patient. This will create a highly effective device.
CURING EACH DEVICE: While the device hardens, you can feel it cool back to room temperature within 15 minutes from the starting point (depending upon temperature and atmospheric conditions in the room). Do not allow the patient to get up until the device is cool (room temperature) to the touch. If the area underneath the patient is still warm, allow it to cool and harden properly before use. This will result in a secure, comfortable tool to quickly reposition your patient each time they come in for treatment.
